Initial Exam with Dr. Jenkins

Please assist us at the time of your initial visit to the office by providing the following information:

  • Your referral slip and x-rays from your referring dentist. X-rays can either be emailed to our office.
  • A list of medications you are currently taking.
  • If you have dental insurance, please bring any forms or insurance cards with you to the appointment.
  • Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmurs requiring pre-medication, severe diabetes, or hypertension.)
  • We will complete an in depth medical and dental health history and a thorough examination to measure for bone loss, loose teeth, bite, oral cancer screening, TMJ and other signs of periodontal disease.

Insurance and Payments

Financial Policy:   The following is a statement of our Financial Policy which we require all patients read and sign prior to any treatment including your initial exam. 

Payment for Services: Patient Payments are due on the day of service. We accept cash, check and credit card (Visa, Mastercard, Discover and Amex.) We also offer Care Credit for those who qualify. Care Credit is able to offer a full range of NO interest payment plans for your dental treatment. Please ask the front office if you are interested in Care Credit and would like more information.

Initial Visit: All new patients referred to our office are scheduled for a comprehensive oral examination with Dr.Jenkins. This may or may not be covered by your insurance. Current dental x-rays will be required in order to provide an accurate diagnosis.  We will do our best to get them from your referring providers office. If they are not available new ones may need to be taken. Once again these may or may not be covered by your insurance. 

Insurance: We will submit a claim to your insurance company on your behalf. It is your responsibility as the patient to be aware of your insurance benefits, exclusions, and frequency limitations. Every plan is different, and changes do occur frequently.  Dr. Jenkins will tailor your treatment plan based on what your treatment needs are. We will submit a prior estimate to your insurance company after your initial visit. This is to provide you with an estimate of what the out-of-pocket cost of your treatment may be and if the treatment is a covered benefit by your insurance.

Private Pay Patients: Full payment is due at the time of your appointment or treatment for individuals not carrying any dental insurance. We offer a 5% savings to those who pay with a check or cash.

Failed or Missed Appointments:  Due to the individualized nature of the treatment we provide, our dental staff prepare treatment rooms and services in advance for each patient. If you are unable to keep your appointment, we require a 24 hour notice prior to your scheduled appointment . We reserve the right to charge a $100.00 fee for any missed appointments or appointments cancelled or rescheduled with less than 24 hour notice.
